| REGISTRATION INFORMATION Conference registration includes conference materials, admittance to Conference Exhibits, continental breakfast, refreshments breaks, light lunch and reception events (Welcome Festival and /or Social Reception) for the day(s) you have registered to attend and a number of opportunities for you to meet informally with other attendees, presenters, sponsors and exhibitors. Reception events include hors d'oevres and no host bar. Parking charges are not included in the registration fees.

We are not responsible for faxed registrations received after February 14, 2007 or not processed due to incorrect credit card information.

Payment Options Payment is required in full when registering. Include a cheque or money order in Canadian funds payable to Pacific Coast Brain Injury Conference or correct credit card information (MasterCard of Visa). Regrettable we cannot accept cheques in US funds. We are not responsible for registrations received after Wednesday, February 14, 2007 or registration forms not processed due to incorrect payment and/or credit card information. Registrations are not processed until full payment is received.
Cancellation/Substitution Policy: All refund or substitution requests must be received in writing. Cancellations received or post-marked by January 15, 2007 will receive a 50% refund to be mailed after the conference. No refunds are given after this date; however, delegate substitutions are permitted.

GENERAL INFORMATION
Name Badge Policy To gain easy access to exhibit hall, conference sessions, meal functions and social events attendees are required to wear their identification name badge throughout the Pre-Conference Day and Conference.
Continuing Education Units/Credits We have applied for CCRC, CRC, RRP and CLCP credits to be issued for the Pre-Conference Day and for each day of the conference. More information is available at the conference registration desk.

The Survivor and Family Lounge A hospitality lounge is available for survivors of brain injury, their family and their caregivers. You are welcome to use the lounge as a quiet place to enjoy refreshments, relax and organize your conference plans. The lounge is open during conference program hours.
